Yes, the one in digipack with promo sticker on back and another one in slimline jewel case!

Answer from a seller when I asked if is a picture disk and if he got it from Eddie..

yes, he and his family are part-time residents in hawaii...he
personally donated these and a few more items for a fundraising
auction for a friend with cancer (see http://www.jeanniechesser.com)
and our environmental group (see
DefendOahuCoalition.org)...there really wasn't the right audience
for the items so we decided to give the fans a shot on ebay...

I was told that there were only 200-300 hundred of these picture vinyls produced and that they are not available to the
public...
